Campaign Disclosure Form The RCA has adopted RCA Board Resolution 08-002 on March 31, 2008 that instituted a process,
procedures, and a form to be used for disclosure by parties, participants, or their agents in
identifying to regular and alternate RCA Board Members contributions that were received from the
Parties. This process and procedure will aid regular and alternate RCA Board Members in meeting
the requirements of Government Code 84308.
Participating Special Entity Application Form The Participating Special Entity Application is used by regional facility providers, such as utility companies, a
public district or agency, that operates and/or owns land within the MSHCP Plan Area that needs to apply for Take
Authorization pursuant to Section 11.8 of the Agreement.
